What is HSS?
Housing Stabilization Services (HSS) is a Medical Assistance program designed to help individuals with disabilities, mental illnesses, or other significant barriers find and maintain stable housing. It provides support to people who are homeless, at risk of homelessness, or in need of assistance to retain their current housing.
Who qualifies for HSS?
To qualify for Housing Stabilization Services (HSS), patients must be enrolled in Medical Assistance and have a documented disability, chronic health condition, or significant limitation affecting their ability to secure or maintain stable housing. Eligible patients may face housing-related challenges such as homelessness, risk of homelessness, living in unsafe or unstable housing, or transitioning from institutional settings like hospitals or shelters. Additionally, they must demonstrate a need for assistance in managing housing responsibilities, such as finding housing, maintaining lease agreements, or budgeting. A person-centered housing plan, developed with a qualified housing provider, is also required to outline specific needs and how HSS can address them.
